Safety culture means paying constant attention to the 3 pillars

The 3 pillars of safety – technical safety, management systems, and human and organizational factors (HOF) – are the 3 levers of safety culture. However, it can be very tempting to address each one separately, in silos, without ensuring that they are consistent when taken together.
